The NHS "normal" testosterone range, roughly 8 to 29 nmol/L, wasn't built to tell you whether you're functioning well. It was built to flag clinical illness. If your GP says your level is "within range" but you feel materially worse than you did five years ago, the range isn't lying. It just isn't answering your question.
What "normal" actually means (without the textbook)
A man brought me his GP results recently. Total testosterone 12 nmol/L. "Within normal range." His GP had told him so.
So I asked him the one thing his GP hadn't.
What's the range?
8 to 29 nmol/L.
Most men assume that's a healthy band. It isn't. It's a statistical band: the spread you'd get if you took blood from every adult man between 18 and 70 and drew a line at the 2.5th and 97.5th percentile.
Athletes. Sedentary. Lean. Overweight. 22-year-olds. 68-year-olds. All in one pot.
The range was built to answer one question: is this man clinically ill?
It wasn't built to answer: is this man well?
Those are two different questions. The NHS test answers one of them. Most symptomatic men are asking the other.
The NHS range of 8 to 29 nmol/L spans roughly a 260% difference between the low end and the high end. Two men, one at 9 nmol/L, one at 25 nmol/L, both receive the same result: "within normal range."
Clinically, they are not the same. The man at 9 nmol/L is statistically far more likely to report fatigue, low libido, and slow recovery. The man at 25 nmol/L is unlikely to report any of those. Both are categorised as normal.
Why the range is so wide
Three things make the reference range as wide as it is. None of them are about whether UK men are healthy.
Age: the biggest single driver
Testosterone peaks in men in their early 20s. From the age of 30, it drops by roughly 1 to 2% per year.
By 45, the average man has lost 15 to 30% of his peak T.
By 60, more than 40%.
That decline is baked into the reference range. The sample includes 18-year-olds at peak and 70-year-olds in slow decline. When you draw a line at the 2.5th and 97.5th percentile across that whole pot, what you get is a span that accommodates physiology decades apart.
That's why the range is wide. It's not because UK men are healthy. It's because the range was built to fit men of all ages.
The obesity shift across the UK population
Adipose tissue contains an enzyme called aromatase. Aromatase converts testosterone into estradiol. More body fat means more conversion. Lower testosterone is the result.
UK obesity prevalence has nearly tripled since 1980.
The reference range gets resampled over time. A pool sampled today contains a much higher proportion of men with metabolic syndrome and visceral adiposity than the pool sampled in 1990.
When the population shifts, the reference range shifts with it. "Normal" today is lower than "normal" was thirty years ago.
Who else is in the sample
Sedentary office workers. Endurance athletes. Men with diabetes. Men with metabolic syndrome. Men on no medications. Men on five.
All in one pot.
The reference range tells you where most men sit. It doesn't tell you whether most men are well.
Including older men with naturally declining testosterone in the same reference population as 22-year-olds mechanically lowers what the system calls "normal." The range itself is a moving target.
The clinical threshold vs the optimal range
The NHS clinical threshold for testosterone deficiency, hypogonadism, sits at around 8 nmol/L. Below that, most GPs will refer to endocrinology for further investigation. Above that, the system mostly leaves you alone.
Three bands matter.
Below 8 nmol/L: the referral line
The NHS refers to endocrinology. Testosterone replacement therapy may be considered as part of a clinical conversation, through the NHS or a private specialist. Andro Prime doesn't sit in that conversation. We're a wellness platform. TRT decisions are clinical, and they belong with a clinician.
8 to 12 nmol/L: the grey zone
The guidance gets unclear. Some clinicians investigate further. Others note the result as "borderline normal" and suggest lifestyle changes.
Most men sitting at this level, who present with classic low-T symptoms, are told they're fine.
Above 12 nmol/L: "fine" by the framework
The overwhelming majority of GPs will categorise this as normal and take no further action, regardless of symptoms.
If you're at 13 nmol/L with a calculated free testosterone in the lowest decile for your age, the NHS framework still calls it normal.
The framework isn't lying. It's just not built to answer the question you're asking.
A 2010 study examined testosterone levels and symptom prevalence in 3,369 European men aged 40 to 79.
The probability of experiencing three or more classic hypogonadal symptoms (fatigue, low libido, erectile dysfunction) increased significantly at testosterone levels below 11 nmol/L.
Eleven. Not eight.
The NHS referral threshold sits 3 nmol/L below where symptoms become statistically likely. Men sitting between 8 and 11 nmol/L are far more likely to be symptomatic than men above 15, but under current NHS guidance, they don't qualify for treatment.

Normal testosterone levels by age (and why there is no clean chart)
People search for a "testosterone levels by age chart". Here is the honest answer: a precise, age-banded "normal" chart does not really exist, and that is the whole point of this article. The NHS reference range (roughly 8 to 29 nmol/L) is a single band that is not adjusted for age. What does change with age is the typical level itself.

These are population averages, not personal targets. Two men of the same age can sit far apart and both be "within range". That is why a single number against one wide band tells you so little, and why a baseline you can retest matters more than where you fall on a chart.
A note on units: UK labs report testosterone in nmol/L. US sources use ng/dL (and supplement sites often quote ng/mL). If you are comparing a US chart to a UK result, the scales are different, so check which unit you are reading before you panic about a number.
What this means if your number came back "normal"
You've had the test. The result is "within range." Something still feels off.
Three possibilities.
You might genuinely be fine. Most men with normal results are. Symptoms can have other causes: sleep, body composition, training load, mood, life.

You might be in the 8 to 12 grey zone with the symptom profile Wu's team found. The NHS framework calls you normal. The evidence base says symptoms become statistically likely below 11.
You might be above 12 nmol/L but in the lowest decile for your age cohort. The framework calls you normal. Your body might disagree.
The framework wasn't built to answer that. The blood test was, provided the test measures more than the headline number.
What changes when you have the full panel (Total T, SHBG, Albumin, and a calculated free testosterone) is the loop. Baseline. Adjust one variable: sleep, training, body comp, alcohol. Wait 8 to 12 weeks. Retest. See whether the number moves.
A single reading is rarely actionable. A number you've watched move is.
