An inflammatory markers blood test measures the chemicals your body releases when something is irritating it. The two that matter for most active men are hs-CRP and ferritin. If your recovery has slowed and sleep and diet haven't changed, the test tells you whether there's a measurable signal behind what you're feeling; and gives you a baseline you can actually retest against.

What "inflammatory markers" actually means (without the textbook)

You probably didn't come here for a biology lesson. You ended up here because something in your body has changed.

The session you'd shake off in a day now takes three. The post-gym ache lingers. Sleep is the same, diet is roughly the same; but the body isn't.

That's the situation a lot of men in their late 30s and 40s land in. Everything looks normal on paper. The mirror says fine. The training log says fine. But the recovery has gone somewhere.

Every one of them got the same question from me.

What did your GP actually test?

The honest answer was usually: not this.

Inflammation is your immune system's response to irritation. In short bursts it's useful: it heals cuts, fights off colds, helps muscle adapt to training. The problem is when it doesn't switch off. Low-grade inflammation that sits in the background for weeks drains energy, recovery and mood in men who otherwise look healthy on paper (NHS, Blood tests overview).

Blood doesn't measure how you feel. It measures the chemicals your body releases when it's responding to something. An inflammatory markers blood test picks those up.

The point of testing isn't to confirm you're struggling. You already know that. It's to find out whether there's a measurable signal underneath it.


The four markers a UK men's blood test would measure

Four markers worth knowing. Two you actually want measured. Two for context, so when a hospital report comes back you know what you're reading.

hs-CRP: the headline one

hs-CRP stands for high-sensitivity C-reactive protein. It's a protein your liver releases when your immune system is busy.

The "high-sensitivity" part matters. Standard CRP picks up obvious infection: flu, abscess, something acute. hs-CRP picks up the low-grade readings that sit underneath active recovery and long-term cardiovascular risk in otherwise healthy men. Ferritin: inflammation in disguise

Ferritin is the protein your body uses to store iron. That's its main job.

It has a second job. It rises sharply when there's inflammation around. Doctors call it an acute-phase reactant. The plain version: this number jumps when the immune system is working.

That second job is what makes ferritin confusing. A man with high ferritin and low iron stores looks contradictory on a basic test. The iron's genuinely low. The ferritin's being pushed up by inflammation. Without the context, you'd miss it.

ESR: older, slower, less useful

ESR (erythrocyte sedimentation rate) is the original inflammation test. Still on most NHS panels because it's cheap. Slower to change than hs-CRP. Less precise for the low-grade picture an active man is trying to read. If your GP test came back with an ESR result, that's normal. hs-CRP is the more useful number.

IL-6 and fibrinogen: research-grade, rarely consumer-tested

IL-6 and fibrinogen are hospital and research markers. Expensive to run. Don't add much that hs-CRP and ferritin don't already cover. Worth knowing the names exist. Not worth chasing on a UK panel.

What counts as "high": the UK numbers

Numbers without context are useless. Here are the thresholds UK private labs and most GPs work from.

hs-CRP risk strata (AHA framework, used by UK private labs)

Under 1.0 mg/L: low cardiovascular risk profile. This is where you want to sit.

1.0 to 3.0 mg/L: average. Common in the UK adult population. Not a crisis but not a clean read either.

Over 3.0 mg/L: elevated. Worth understanding why, and worth retesting in 4 to 8 weeks.

Over 10 mg/L: your GP that week. This range points to acute infection or a more significant inflammatory event, not lifestyle drift.

The AHA's three-band framework (under 1 / 1 to 3 / over 3 mg/L) is the de-facto UK standard. NHS clinical guidance references the same cutoffs (Pearson et al., 2003, AHA/CDC Scientific Statement in Circulation, Markers of inflammation and cardiovascular disease).

One reading on its own is rarely actionable. A hs-CRP taken the morning after a hard session, the week of a cold, or shortly after dental work can sit elevated for reasons that have nothing to do with your baseline.

The practical rule: don't decide off a single number. Test. Wait 4 to 8 weeks. Retest when you're not acutely ill and haven't trained brutally in the previous 48 hours. The number you've watched move is the one that means something.

Ferritin's two-direction problem

Ferritin matters at both ends, which is why it gets confusing.

Low ferritin, under 30 µg/L in adult men, means depleted iron stores. If recovery is slow, sleep is fine, and ferritin is low, that's a plausible cause. It's a GP conversation, not a supplement one. Iron has a real overdose risk. It needs to be dosed properly.

High ferritin can mean inflammation pushing the number up. Or heavy alcohol load. Or, much more rarely, genuine iron overload (haemochromatosis). It's a signal, not a verdict. Your results may show a single elevated ferritin reading that isn't actually about iron at all.

What high inflammation usually means in active men

If you're an otherwise healthy man between 35 and 50, persistently elevated hs-CRP is almost never the dramatic story. It's a slow accumulation.

Most of the time, it's one of five things. Sometimes two of them stacked.

Training load that's outrun recovery

The most common cause in men who train. Sessions get harder. Weekly volume creeps up. Deloads get skipped. The body never gets the recovery week it actually needs.

A 2020 systematic review in Frontiers in Physiology found measurably elevated baseline CRP in men running heavy training volume, with the effect largest in those at 8+ hard sessions a week without structured deloads (Cerqueira et al., 2020, Inflammatory effects of high and moderate intensity exercise).

What it looks like in practice: hs-CRP that sits at 2 to 4 mg/L across a deload week, not just after a hard session. The post-session bump is normal. The persistent elevation is the problem.

Sleep debt

Sleep isn't a soft variable.

Chronic short sleep, under 6 hours for several weeks, raises inflammatory markers by a measurable margin. Cohort studies put the effect somewhere in the order of 30 to 40%, with hs-CRP specifically affected (Irwin et al., 2016, Biological Psychiatry).

Quality matters too, not just hours. A consistently broken seven hours can sit worse than a clean six.

Body composition and visceral fat

Visceral fat is the fat that sits around the organs. Not the fat you can pinch.

It's metabolically active. It releases inflammatory signals on its own. That's why men carrying weight around the middle often run elevated hs-CRP even when the rest of the lifestyle looks reasonable.

It's also one of the most responsive levers. A 5 to 10% reduction in body weight is consistently associated with a meaningful drop in hs-CRP.

Alcohol and ultra-processed food

Both raise the noise floor.

Heavy alcohol use, more than 14 units a week (the UK guideline), drives inflammation directly. Ultra-processed food is more indirect: gut effects, blood sugar swings, the trans fats some of it still contains. The signal is real.

These are the two cheapest levers to pull. They're also the two most often dismissed.

The other half: when it isn't lifestyle

You've genuinely checked all four of those for eight weeks. The number won't come down. The answer probably isn't more discipline.

It's a different conversation.

Common other causes: a dental infection (a low-grade abscess can run hs-CRP up for months without much pain). Gut issues. An undiagnosed autoimmune condition. A long-tail post-viral picture. None of these are things you fix with a better protein shake. They're things you take to a GP.

The point of testing isn't the number. It's the loop.

Without a baseline, you're guessing. You add fish oil. You cut the lunchtime beer. You sleep an extra hour for a fortnight. And you don't know whether any of it worked.

Can training cause high inflammation markers?

Yes. Hard sessions raise hs-CRP for 24 to 72 hours, sometimes longer after a marathon or heavy resistance block. If you test the morning after a brutal leg day, you'll get a noisy number. Persistently raised hs-CRP across a deload week is a different signal: that's training load that's outrun your recovery, not a normal post-session bump.

How long does it take inflammation markers to come down?

If a single reading is being driven by an acute event (a cold, a hard race, recent dental work) hs-CRP usually settles within 1 to 2 weeks. If the cause is lifestyle (training load, sleep debt, alcohol, body composition), expect 6 to 12 weeks of changes before a retest shows movement. That's why the practical retest window is 4 to 8 weeks at minimum, and we'd usually suggest 8 to 12.

Should I see my GP about high CRP?

Yes for any single reading over 10 mg/L, persistently above 3 mg/L across two retests 8 weeks apart, or any reading alongside unexplained weight loss, night sweats or persistent fever. Can supplements reduce inflammation?

Some lifestyle changes have evidence behind them: sleep, training periodisation, reducing alcohol and ultra-processed food. Can high inflammatory markers cause fatigue?

High markers do not directly 'cause' fatigue, but persistent low-grade inflammation and the tiredness active men describe often travel together. A raised hs-CRP across a deload week (not just after a hard session) is a signal that training load, sleep, body composition or alcohol may have outrun recovery. It is a prompt to look at those inputs and retest, not a diagnosis on its own.

What is the difference between inflammatory markers and infection markers, and what does a very high reading mean?

They overlap: CRP is used both to track low-grade inflammation and to flag infection. The difference is scale. Low-grade, recovery-relevant readings sit in single digits (hs-CRP under 3 mg/L is the target band). Markedly high readings, and certainly a CRP in the hundreds, point to significant infection or acute illness, not lifestyle drift. Any reading over 10 mg/L is a GP conversation; a very high reading needs prompt medical attention, not a wellness blog.
